Child Name

This field lists all children who are members of the adoption case. The name you select here will appear in appropriate fields in the report. This field is required.

From Date

To limit the report to conferences within a specific period of time, enter the earliest date you want the report to cover. Otherwise, leave this field blank.

To  Date

To  limit the report to conferences within a specific period of time, enter the latest date you want the report to cover. Otherwise, leave this field blank.

Print All Matching Conferences for Child

Select this check box to include all matching conferences in the report regardless of date. Do not select this field if you specify a date range.
