A Voluntary Agreement for Care (VAC) is created and implemented when the parents, guardians, or custodians are unable to care for the child and agree to give custody voluntarily to the agency for a period of time that's less than 90 days. The agreement, and the change in legal status, are recorded on the child's person profile (see Record a child's legal status). The system generates one of the following reports, which are displayed on the child's SACWIS History screen (see View a person's SACWIS history):

To generate a VAC report from the child's case overview, complete the following steps:
Click on the Case tab. Select the Workload tab. Select your name link. The cases assigned to the worker are listed below.
Click the appropriate ID Number link. The Case Overview screen appears.
Select the Forms / Notices link in the navigation bar on the left. The Maintain Forms/Notices screen appears.
In the Reports field drop down list, select the appropriate JFS report.
Click the Report button. The Document Details screen appears.
To print an existing report, click the ID link.
To print a new report, click the Generate Report button. The JFS 01645 - Agreement for Temporary Custody of Child screen appears.
Complete the fields on this screen to complete the agreement. Several fields are mandatory.
Click the Generate Report button. The report appears in a PDF file format.
To print the report, select File, and then Print in your browser menu.
To save the report to the report history in Ohio SACWIS, click the Save button in the lower-left area of the screen.
Or, click the Cancel button if you do not want to save the report. You are returned to the Maintain Forms/Notices screen.
If you opened an existing report, close the browser window to return to the previous screen.
