Record Provider Name

Process: Record basic provider information

For an ODJFS provider, the name is usually entered on the original provider inquiry record. It can be updated via the Basic Provider Information link on the home study record, as long as the home study is still in "Pending" status. If the home study is approved, you must create a new home study record. (See Record a home study.) For a non-ODFJS provider, the name can be updated via the Provider Information link on the Provider Overview screen.

The system allows only one current provider name for the provider record. A current record is one that has not been end-dated.

Add or update a provider's name as follows:

  1. From the Basic tab on the Provider Name Information screen, click the Edit link for the name record you want to correct, or click Add Name for a new name record. The Member Details screen appears. It reflects the name you used in your search.

  2. Verify the provider's name in the Provider Name field.

  3. Enter the date this name was in effect in the Effective Date field.

  4. Click OK. You are returned to the Basic tab.

  5. Click Next Task below to continue.
