Add/edit statewide service ceiling

Services Group/ Category

Select the group or category of the service for which you view ceiling costs. This field is required. Once the ceiling has been saved, you cannot change this field.

Begin Date

Enter the date on which this ceiling becomes effective.  Use the format MM/DD/YYYY. If the effective date range you enter here overlaps the date range of an existing record for the same ceiling, the system processes the change as an adjustment. This field is required.

Ceiling Type

Select the type of the ceiling you are entering. This field is required. Once the ceiling has been saved, you cannot change this field.

End Date

Enter the date on which this ceiling is no longer effective.  Use the format MM/DD/YYYY. The effective date range you enter here cannot overlap the date range of an existing record for the same ceiling. This field is required.

Ceiling Amount

Enter the amount of the ceiling. The ceiling may be expressed as dollar amount of as a percentage. (You must indicate which.) This is a required field.

Dollar

Click this button if the ceiling amount is expressed as a dollar amount.

Percentage

Click this button if the ceiling amount is expressed as a percentage a dollar amount for reimbursement.
