This screen allows you to record the basic information collected during an intake. Intakes that come into the agency are recorded by a screener, or screening decision maker who collects the information and records it in Ohio SACWIS. Intakes may be in the form of telephone calls, letters, face-to-face visits, faxes, email, or any other electronic method. Depending on the details recorded on this Basic tab screen, the system will require additional fields and tabs be completed in the intake report. (See About intakes.)
Referrals fall into an Intake Category of:
Child Abuse/Neglect (CA/N)
Family in Need of Services (FINS)
Information and/or Referral (I and/or R)
Dependency
Based on the category selected; the system will then generate Intake Types for selection. By clicking on the Other Intake Designations link a list of other available designations displays based on the category.
Below the intake category, and types; is a list of questions. This section of the screen is dynamic meaning additional fields will display for more information based on the answers to these questions.
For the questions, Does this report require a Specialized Assessment/Investigation; if the answer is Yes, an additional tab will display within the intake report called Specialized.
At the bottom of the screen, there are optional radio buttons for you to select screening priority for the intake report. The options are None Specified; Low, Med, and High.
Once the intake decision has been saved the information on this screen is read only.
