This screen allows you to view, add, or update any medical, mental, or dental treatments this person has received, including any hospitalizations. The sections that appear on this screen depend on the selections you made on the Medical Records screen.

Fields on this screen

Provider Name	Click Search to find the provider in the system. Providers are added to the system in the Provider area. This field is required.
Treatment Type	Select the type of treatment this person received. This field is required.
Primary Service Type	Select the specific service this person received. This field is required.
Other	If you selected Other in the Primary Service Type field, describe the type of treatment. You can enter up to 25 characters. This field is required when you select Other.
Secondary Service Type	If the person received another service as part of this treatment, select that service here.
Other	If you selected Other in the Secondary Service Type field, describe the type of treatment. You can enter up to 25 characters. This field is required when you select Other.
Service Start Date	Enter the date the person received the service. Use the format MM/DD/YYYY. This field is required.
Service End Date	If the service took place over a period of time, enter the date the service was completed. Use the format MM/DD/YYYY.
Hospitalization	Indicate whether hospitalization was required.
Hospital Name	Enter the name of the hospital. You can enter up to 90 characters.
Date Admitted	Enter the date the person was admitted to the hospital. Use the format MM/DD/YYYY.
Date Discharged	Enter the date on which the person was discharged from the hospital. Use the format MM/DD/YYYY.
Treated By	Enter the name of the physician of record, that is, the physician who treated the person.
Surgery	Select Yes, No, or Unknown to indicate whether the person underwent surgery.
Hospitalization Details	If the child was hospitalized, enter a reason and any other relevant information. You can enter up to 3,000 characters.
