Immunization Type	Select the immunization the child received. This field is required.
Other Immunization Type	If you selected Other in the Immunization Type field, describe the immunization the child received. You can enter up to 50 characters. This field is required when you select Other.
Date of Immunization	Enter the date the child received the immunization. Use the format MM/DD/YYYY. This field is required.
