Whole/Term

Select the radio button next to the type of insurance policy.

Name of Company

Enter the name of the company that holds the insurance policy. You can enter up to 40 characters. This field is required.

Effective Month/Year

Enter the month and year when this policy was purchased. Use the format MM/YYYY. This field is required.

Policy Number

Enter the insurance policy number. You can enter up to 16 characters. This field is required.

Policy Value

Enter the value of the life insurance policy. This field is required.

Cash Surrender Value

Enter the cash surrender value, which is based on the life of the policy. Obtain this value from the insurance company. This field is required.

Loan Amount

If the person has a loan against this policy, enter the amount of the loan.

Description

Enter a description of the insurance policy or loan.

Invalidate Record

Select the check box if this record was created in error. (You cannot delete a financial profile record.) An invalid record cannot be updated. Clear the check mark to return this record to valid status.

Reason

Enter a reason for invalidating the record or returning it to valid status. You can enter up to 400 characters.
