Minimum Age

Enter the youngest age the provider is willing to accept.

Sibling Group

Indicate whether the provider is willing to accept sibling groups.

Gender

Select the gender the provider is willing to accept.

Maximum Age

Enter the oldest age the provider is willing to accept.

Minor Mother

Indicate whether the provider is willing to accept a minor mother.

Accept More Than 1 Child

Indicate whether the provider is willing to accept more than one child.

Race

Select the check boxes next to each race the provider will accept. If you select Multiracial, enter a brief description of the resource's preference. You can enter up to 60 characters in the Description field.

Available Ethnicity

Select each ethnic origin the provider will accept, then click Add>>.

Selected Ethnicity

This field reflects the ethnic origins selected in the list box on the left. To remove an ethnic origin, select it, then click <<Remove.

# of Bedrooms

Enter the number of bedrooms in the provider's home.

Marital Status

Select the marital status of the provider.

Transportation Available

Indicate whether the provider has a car, bus pass, or other type of transportation.
