Member details (provider)

Member Name

This is the staff member you selected or added on the Current Active Members screen.

Member Role

Select the person's role. This field is required.

Effective Date

Enter the date on which the role was effective. Use the format MM/DD/YYYY. This field is required. You cannot enter a date in the future.

End Date

Enter the date on which the member left the provider's staff or household. Use the format MM/DD/YYYY. You cannot enter a date in the future.

Reason

Select the reason why the person left the provider. This field is required if you enter a date the End Date field.
