Maintain transactions screen

This screen allows you to view, add, or update the details of a recommendation for approval or certification. As the home study assessor, you record a recommendation for approval or certification once a home study is complete.

You can update a recommendation only if it is in "Pending" status. The approver can update a recommendation that has been routed only by opening the record from the Review link on the Maintain Approval/Certification Recommendations screen.

All of a provider's facilities from the last certification are included in an application for recertification. (These facilities may be unlinked from the provider record, however.)

The Process Approval button appears on this screen before the recommendation has been routed. It allows the assessor to route the recommendation for approval.

Fields on this screen

Agency Contact Person	Select the worker who should be contacted about this recommendation. This list displays the workers who are currently assigned to this provider record. This field is required.
Provider Type	Select the type of provider. If this recommendation record is created from a home study, this field displays the provider type selected on the home study. This field is required.
Transaction	Select the type of recommendation or other transaction. This field is required.
Recommendation Date	Enter the date of this recommendation. Use the format MM/DD/YYYY. This date must be: after the effective date on the provider type record the same as or after the approval/ certification effective date before the approval/certification expiration for all transaction types except Recommendation for Initial Certification, Recommendation for Recertification, Initial Approval or Update
Name Change	Select this check box to indicate the provider's name has changed. This field is enabled when you select Change in Circumstance in the Transaction field.
Level of Care Change	Select this check box to indicate that the provider's level of care has changed. This field is enabled when you select Change in Circumstance in the Transaction field.
Marital Status	Select this check box to indicate that the provider's marital status has changed. This field is enabled when you select Change in Circumstance in the Transaction field.
Relocation	Select this check box to indicate the provider has relocated.
Closed Reason	If the transaction type is Recommendation for Closure, select the reason for closure. This field is enabled when you select Change in Circumstance in the Transaction field.
If Other, Explain	If you selected Other in the Closed Reason field, enter a detailed explanation for the closure. You can enter up to 2,000 characters.
Receiving Agency	If you selected Transfer in the Transaction field, select the agency that will receive this provider.
Receiving Agency Contact Person	If you selected Transfer in the Transaction field, select the worker who is the main contact person for this provider. This displays the workers at the agency to which you are transferring this record.
Comments	Enter any additional information about this transaction. You can enter up to 2,000 characters.
