Record Personal Characteristics

Process: Record a person profile.

The Characteristics tab in the Person Profile is where you document clinical assessment details. This is required to support Federal Reporting by allowing the worker to record diagnoses of medical or mental health conditions. The Characteristics tab also provides the ability to record self-reported or observed characteristics of a person. These characteristics details may assist the worker when making placement decisions for a child.

Conduct a Person Search. To record personal characteristics, from the Person Overview screen, complete the following steps:

  1. Click the Characteristics tab. The Documented Person Characteristics screen appears.

  2. IF applicable, select one of the two check boxes:

A clinical assessment by a qualified medical or mental professional has not yet been completed for this person

A qualified professional has conducted a clinical assessment of this person and has determined this person has no clinically diagnosed conditions (Medical, Mental Health/Substance Abuse, Prenatal/Birth, or Developmental/Intellectual).

  1. In the Person Characteristics area, click in the Characteristic Type drop down list.

  2. Select the characteristic type that applies. For diagnosed characteristic types, select:

Developmental/Intellectual

Medical

Mental Health/Substance Abuse

Prenatal/Birth

  1. Click the Add Characteristic button. The Characteristic Details screen appears.

  2. Under the Characteristics Group heading, select all Available Characteristics that apply. Click the Add link. The selections appear in the Selected Characteristics table. To remove; select all that you want to remove and click Remove.

  3. Select the Method radio button that applies:   Unknown    Self Reported    Observed    Clinically Diagnosed

To meet SACWIS/Federal Requirements for children in the system, the method of Clinically Diagnosed must be selected, with the name of the doctor making the diagnosis, and the date of the diagnosis. If the child's clinically diagnosed condition ends, you will enter the End Date on this screen in the End Date field.

  1. Although not required; enter Additional Information if available.

  2. Click the Save button. The Documented Person Characteristics screen appears. The characteristics you added appear in the Person Characteristics table.

  3. Record undiagnosed person characteristic details as needed.

  4. Click Next Task below to continue.
