Add a Kinship Provider

In Ohio SACWIS, a kinship provider is a type of ODJFS provider. Like any ODJFS provider, you must enter a provider inquiry first. Provider records are created from the inquiry. Then you can update the provider record as necessary and complete the service credential record.

Add a kinship provider as follows:

1. Record the provider inquiry.

   • On the inquiry record, select one of the kinship values in the Inquiry Type field. This will become the provider type.

   • As with any inquiry, you must record activities "Application Provided" and "Application Received" before you can complete the inquiry. If you are bypassing the application for this kinship provider, you still must add these activities, but enter in the narrative field a comment that no application was given or received.

   • If a provider record already exists for the family, you will link it to the existing record. If the family is new to Ohio SACWIS, you will link the inquiry to a new provider record.

2. Complete or update the provider's basic information.

   • If you are creating a  new provider, the system displays the Basic Information tab on the provider record. Note that the provider type is the same as the inquiry type, which should be one of the kinship values. The provider status is "Active." The address is the one entered on the provider's person profile record. Update the information as necessary. See Record basic provider information.

   • If this is an existing provider, the system displays the Provider Overview screen. Make updates to basic provider information from the Basic Information link in the Provider Actions area on this screen. Make sure the provider type on the Basic Information tab reflects one of the kinship values (see Record provider type), the type status is "Approved," and the provider status is "Active" (see Record provider status).

3. Complete the provider service credentials. See Record a service credential (other services).

   • The service category is "Placement," the service type reflects one of the kinship care values, and the service status is "Active."

4. Once you have completed the kinship provider record, you can add the placement record for the child. See Record placement information.
