Activity log filter criteria (provider) screen

This screen allows you to view or add items in the provider's activity log. You can use a variety of search criteria to filter the display of activity log items. The search results list at the bottom of the screen displays restricted activity log items only if you are assigned to the provider record.

The Edit link allows you to update an activity record in "Draft" status. The View link allows you to open the record in read-only mode. The Add Activity Log button allows you to create a new activity record.

The Report link allows you to print a single activity log item. The Print Current Page button allows you to print in one report the activity log items currently displayed in the Result(s) table. If you want to print all activity log items, you must display them in the table before clicking the Print Current Page button. That is, you may need to generate several reports for all the activity log items.

Activity From Date	To find activity log items recorded within a certain date range, enter the beginning date of the range. Use the format MM/DD/YYYY.
Activity To Date	To find activity log items recorded within a certain date range, enter the ending date of the range. Use the format MM/DD/YYYY.
Responsible Worker	To include the responsible worker in your search criteria, select the worker's name. The system displays the names of all workers who have been assigned to this provider.
Contact Type	To include the contact type in your search criteria, select the contact type.
Category	To include the category of activity in your search criteria, select the category.
Sub-Category	To include the subcategory of activity in your search criteria, select the subcategory.
Created By	To include the worker who created the activity log item in your search criteria, select the worker's name. The system displays the names of all workers who have created an activity log item for this provider.
Activity State	To include the activity state in your search criteria, select the state.
Sort Results By	Select the method by which you want the system to display your search results.
