Activity log details (provider)

Responsible Worker

Select the name of the worker responsible for this activity. This list displays the workers who have been assigned to this provider. This field is required.

Originator of Information

Enter the name of the information source. You can enter up to 50 characters.

Activity Start Date

Enter the date on which this activity started. Use the format MM/DD/YYYY. This field is required. The system defaults to today's date.

Start Time

Enter the time the activity started. Use the format HH:MM.

Activity End Date

Enter the date the activity ended. Use the format MM/DD/YYYY.

Activity End Time

Enter the time the activity ended. Use the format HH:MM.

Contact Type

Select the method by which the contact was made. This field is required.

Contact Duration

Enter the length of the contact. Use the format HH:MM.

Category

Select an activity category. This field is required.

Sub-Category

Select a subcategory of activity. This field is required.

Other Sub-Category

Select the subcategory, if any,  that further distinguishes the activity.

High Priority

Select this check box if this activity has a high priority. When you select this, the system sends an email notification to all workers assigned to the provider. The notification instructs the worker to review the activity log item.

Restricted

Select this check box to indicate that this activity log item is restricted. Only the workers assigned to this provider can view restricted activity log items..

Location Type

Select the location type for this activity.

Other Location

If you selected Other in the Location Type field, enter a brief description of the location. You can enter up to 50 characters.

Location Details

Enter a description of the physical location or directions. You can enter up to 200 characters.

Save Note to Child(ren)
Record

Select this check box to add this activity log item to the child's case notes record.

Narrative

Enter a detailed description of the activity. You can enter up to 4,000 characters. This field is required.

Activity State

Select the state of this activity log item. This field is required.

When an item is in Draft state, you can edit any of the fields. When you select Completed and save this screen, no changes may be made to this activity log record, except corrections in the Narrative field.
