In Ohio SACWIS, an ODJFS provider is a foster or adoptive home provider. The provider record is initiated through the inquiry process. Once the provider record is created, additional information is collected through the home study. Once the provider has been approved or certified and the service credentials have been activated, the provider is included in the client match process, and children may be placed with the provider.
Add an ODJFS provider as follows:
Complete or update the provider's basic information.
If you are creating a new provider, the system displays the Basic Information tab on the provider record. Note that the provider type is the same as the inquiry type. The provider status is Active. The address is the one entered on the provider's person profile record. Update the information as necessary. See Record basic provider information.
If this is an existing provider, the system displays the Provider Overview screen. Make updates to basic provider information from the home study record. See Record a home study.
Complete the home study. See Record a home study.
Complete the provider service credentials. See Record a service credential (other services).
The service category is Placement. The service type reflects one of the kinship care values, and the service status is Active.
Add the placement record for the child. See Record placement information.
Initiate the approval or certification process. See Record a recommendation for certification or Approve a recommendation for certification.
Set up a payment account for the provider. See Record payment account details.
Activate the service credentials for the services the provider is authorized to provide. See Activate or deactivate provider service credentials.
