Case plan

This screen displays any case plans that have been recorded on this case. It allows you to view, add, or update case plan details. You create a case plan when a decision has been made to provide ongoing services to a family. You create one case plan for a case unless the court directs the agency to create an additional plan.

The case plan in Ohio SACWIS is the tool used for addressing the overall needs of the family or case participants. The case plan is typically recorded after the family assessment has been completed. However, the case plan is created without a family assessment when the case plan participant is a deserted baby, safe-haven child, an emancipated youth, or a permanent-surrender child.

If a child is in the permanent custody of the agency, a child-only case (adoption case) is created to accommodate a child-only case plan. If a sibling group that is eligible for adoption is documented in the adoption case, one case plan can be developed for all siblings.

The view link allows you to open the case plan in read-only mode. The Edit link allows you to update a case plan that has not been approved yet. The copy link allows you to create a new case plan by copying an approved plan. The Reports link allows you to print the JFS 014410 Maintain Case Plan or JFS 01445 Case Plan Amendment documents.

The amend link allows you to record the details of any amendment to a case plan. The history link displays a history of the case plan creation and amendments. The Court/Signature Details link allows you to record case member signatures and court filing dates.
