When a child is placed with a relative, a home (ODJFS) provider record is created for the relative. (See About provider information.) This provider type is "Kinship Care" (relative or non-relative). If the provider record was entered in Ohio SACWIS previously but was closed, you can reopen the provider record as a kinship provider.
Reopen the provider record as follows:
Create a new inquiry record.
When you identify the provider on the inquiry, you retrieve the provider's original person profile. The provider has the same person ID number.
Link the inquiry to the existing, closed provider record. After you click Save, the Provider Overview screen appears.
Update the provider record as necessary, including the following:
Address and contact information
Usage placement criteria
Home description
Family description

Once these details have been updated, update the provider type status.
If service credentials for kinship care were entered originally on the provider record, reactivate the services.
If service credentials for kinship care were not entered before, record these services as "other" services.
Create a new activity log entry.
In this entry, describe any changes that have occurred since the provider was previously active. Also explain what steps the agency has taken to assure that this provider is a suitable placement. Add any other information, as required, by your agency policy on kinship care. For example, you would create activity log records to describe the completion of BCII and/or FBI background checks.
Click Save. You are returned to the Provider Overview screen.
